A worker, employed by Norfolk Disposal Services Limited of Waterford, Ontario was fatally injured after a waste collection truck tipped over on a collection route. The company failed to provide information, instruction and supervision to a worker to ensure they were able to operate the vehicle safely, as required by section 25(2)(a) of the Occupational Health and Safety Act, contrary to section 66(1) of the Act.

Description of Offence: Two workers were injured when a section of an unbraced masonry block wall collapsed.

Fortis Construction Group Inc. failed, as a constructor, to ensure that the wall was adequately braced as required by section 31(1)(b) of Regulation 213/91, a violation of section 23(1)(b) of the Occupational Health and Safety Act.

Blog Post #1614 – Workplace Fatality Results in $100,000 Fine for London Company

A worker, employed by Integrated Solutions Inc., of London, Ontario, a company that installs satellite dishes on residential roofs, was fatally injured by a fall while installing a satellite dish on the roof of a residence. By not ensuring the worker used fall protection while on the roof, Integrated Solutions Inc. failed to take every precaution reasonable in the circumstances for the protection of a worker, contrary to section 25(2)(h) of the Occupational Health and Safety Act.
