A worker, employed by E.H.E. Construction Group Inc. of King, Ontario, was critically injured when a section of a concrete basement wall collapsed at a residential construction project. E.H.E. Construction Group Inc. failed to ensure that the measures and procedures prescribed by section 31(1)(b) of Ontario Regulation 213/91 were carried out at the workplace, contrary to sections 25(1)(c) and 23(1)(c) of the Occupational Health and Safety Act.

The company failed, as an employer, to ensure that the measures and procedures prescribed by section 150(2)(a) of Ontario Regulation 213/91 (Construction Projects) were carried out, contrary to section 25(1)(c) of the Occupational Health and Safety Act.

A worker, employed by Fer-Pal Construction Ltd., 171 Fenmar Drive, North York, Ontario, a trenchless technology company involved in water main rehabilitation, suffered serious injuries after being exposed to hot water while working on a water main. Fer-Pal Construction Ltd. failed, as an employer, to ensure that the measures and procedures prescribed by section 71 of O. Reg. 213/91: Construction Projects were carried out, contrary to section 25(1)(c) of the Occupational Health and Safety Act. Additionally, Antonio Visconti, as a supervisor, contravened section 27(1)(a) of the Act.

Blog Post #1719 – Toronto Contractor Fined $80,000 After Workplace Fatality

 A worker, employed by KAS Aluminum & Copper Ltd. of Toronto, Ontario, a contractor that installs windows, eavestroughs and siding, was fatally injured falling from a roof. The company failed to ensure the worker was using fall protection when exposed to the hazard of falling more than three metres, contrary to section 26.1(2) of Ontario Regulation 213/91 as required by section 25(1)(c) of the Occupational Health and Safety Act.
