A worker, employed by Sega Group Inc. of Navan, Ontario, an electrical contracting company providing powerline and utility services, was injured when a concrete streetlight post toppled during hoisting. The company failed, as an employer, to ensure that the measures and procedures prescribed by section 150(2)(a) of Ontario Regulation 213/91 (Construction Projects) were carried out, contrary to section 25(1)(c) of the Occupational Health and Safety Act.

A worker, employed by Stobag North America Corporation of Burlington, Ontario, a manufacturer of sun shading systems including awnings, canopies and blinds, sustained critical injuries after a fall from an elevated work platform that was struck by a moving toolbar. The company failed to take every precaution reasonable in the circumstances for the protection of a worker, contrary to section 25(2)(h) of the Occupational Health and Safety Act.

The company failed, as an employer, to provide information, instruction and supervision to a worker to protect the health or safety of the worker as required by section 25(2)(a) of the Occupational Health and Safety Act.
