A worker was critically injured by an automatic roll-down door while operating a lift truck. Nova Cold Logistics ULC o/a Americold failed, as an employer, to ensure the bottom edge sensor on the roll-down H-Door was maintained in good condition, contrary to the Occupational Health and Safety Act.

Misteelco Inc. failed, as an employer, to ensure the beam was handled without endangering the safety of a worker as prescribed by section 45(b) of the Regulation for Industrial Establishments and contrary to section 25(1)(c) of the Occupational Health and Safety Act.

A worker was seriously injured while repairing the brakes on a motor home. Bert’s Truck and Trailer Repair Inc. failed, as an employer, to protect a worker by ensuring the safety measures and procedures required for preventing an elevated vehicle from falling on a worker were carried out, as outlined in section 74 of the Regulation for Industrial Establishments.
