On June 1, 2021, two workers were dismantling a walk-in cooler. One of the workers, who was not wearing fall protection, used a scissor lift elevated work platform to step on the top of the freezer to unscrew metal plates attached to the roofing panels.

A worker, employed by Toronto Hydro-Electric System Limited, Toronto, ON, an electric utility that owns and operates the electricity distribution system for the City of Toronto, suffered a fatal injury while performing work in an underground electrical vault.

A worker was attempting to do a quality check on a vertical wire drawing machine. The machine was located on an elevated platform accessible by a stairway with six steps. While climbing the stairs to perform the task, the worker lost their footing. When instinctively extending their arms to brace their fall, the worker’s gloved hand came in contact with the moving wire, injuring the worker. Indwisco Limited failed to ensure the machine had a guard or other device to prevent access to the pinch points as prescribed.

As the federal government moves forward with pot legalization, the national safety association for the upstream oil and gas sector is demanding the continued prohibition of marijuana use among employees in the industry, citing the drug as a workplace hazard.
