Flooding led to the capsizing of a fishing vessel in 2014, an accident that tossed four fishers overboard and killed one of them, according to an investigation report released by the Transportation Safety Board of Canada (TSB) on August 10, 2015.

A worker employed by L.Z. Refrigeration Inc. was fatally injured at the workplace while operating an Elevating Work Platform (EWP). Yuqin Liu failed as a supervisor to ensure that a worker worked in a manner and with the protective devices, measures and procedures as required by the Occupational Health and Safety Act. They also failed to provide the worker with oral and written instructions or training on the operation of an EWP which includes instructions with respect to the owner’s manual.

During construction of high-rise residential towers, a worker was fatally injured when a thirteen-ton counterweight from an excavator fell on them. Orin Contracting Services Inc. failed to ensure the safety measures and procedures for removing a counterweight from an excavator.

The Ottawa City Council has voted unanimously to petition the Ontario Labour Minister and Attorney General to push for criminal investigations and convictions regarding occupational health fatalities and serious workplace injuries, in accordance with 2004 amendments to the Canadian Criminal Code of Canada, known as the Westray Law.
