Two workers, employed by Aluma Systems Inc. of Bolton, Ontario, were injured when the shoring under a concrete pouring operation collapsed. Aluma Systems Inc. failed to ensure re-shoring was supported and braced appropriately.

A pedestrian was fatally struck by a dump truck in a work zone near a TTC bus stop. D & A Road Services Inc. failed to ensure that measures and procedures under section 65 of Ontario regulation 213/91, which requires a fence to be constructed between the public walkway and the project were carried out in the workplace.

Nearly three months after the death of a sewer worker, the City of Edmonton announced a review of the safety culture of municipal workplaces on January 25, 2017. But a spokesperson says the review, which will examine the three distinct areas of people, processes, and technology, is not a response to the sewer tragedy, but a part of a general safety initiative that the city has already launched.
