A worker, employed by Fer-Pal Construction Ltd., 171 Fenmar Drive, North York, Ontario, a trenchless technology company involved in water main rehabilitation, suffered serious injuries after being exposed to hot water while working on a water main. Fer-Pal Construction Ltd. failed, as an employer, to ensure that the measures and procedures prescribed by section 71 of O. Reg. 213/91: Construction Projects were carried out, contrary to section 25(1)(c) of the Occupational Health and Safety Act. Additionally, Antonio Visconti, as a supervisor, contravened section 27(1)(a) of the Act.

Blog Post #1618 – Waterford Waste Disposal Company Fined $160,000 After Worker Fatality

A worker, employed by Norfolk Disposal Services Limited of Waterford, Ontario was fatally injured after a waste collection truck tipped over on a collection route. The company failed to provide information, instruction and supervision to a worker to ensure they were able to operate the vehicle safely, as required by section 25(2)(a) of the Occupational Health and Safety Act, contrary to section 66(1) of the Act.
