A worker, employed by BNE Concrete Floors & Coatings Inc. of Cambridge, Ontario, was critically injured after prolonged exposure to carbon monoxide. The company failed to ensure that the measures and procedures prescribed under section 47(2) of Ontario Regulation 213/91 were carried out at the workplace, contrary to section 25(1)(c) of the Occupational Health and Safety Act. The company also failed to immediately notify an inspector of a critical injury at the workplace, contrary to section 51(1) of the Act.

BND Woodworking Inc. failed, as an employer, to ensure that equipment, materials and protective devices provided by the employer were maintained in good condition at a workplace, as required by section 25(1)(b) of the Occupational Health and Safety Act.

 A worker was fatally injured due to carbon monoxide exposure following a fire in an oil‐lubricated air compressor. OJB Industries Inc. of Lucan, Ontario, failed, as an employer, to ensure that measures and procedures prescribed by section 10(5) of Ontario Regulation 833, CONTROL OF EXPOSURE TO BIOLOGICAL OR CHEMICAL AGENTS, were carried out at a workplace contrary to section 25(1)(c) of the Occupational Health and Safety Act.
