A worker, employed by the Corporation of the Municipality of Lambton Shores, in Forest, Ontario, was injured while trying to clean a Zamboni ice re-surfacer. The Corporation of the Municipality of Lambton Shores failed to provide information, instruction and supervision to a worker with respect to the safe operation, maintenance and/or cleaning of the ice re-surfacer, contrary to section 25(2)(a) of the Occupational Health and Safety Act.

Contrary to safety procedures, Sin Structures Inc. failed, as an employer, to ensure equipment was moved without endangering the safety of a worker as outlined in the Occupational Health and Safety Act.
