Two workers employed by Gerdau Ameristeel located in Whitby, Ontario were injured after being struck with a piece of waste material that fell from a furnace. There was no overhead protection to protect the workers from the hazards of falling material.

A worker, employed by the Corporation of the City of London, Ontario, was injured when a cap on a drain pipe shot off and struck the worker, resulting in critical injuries. The employer failed to take the reasonable precaution of ensuring that the internal pressure of the drain pipeline was released before the mechanical joint end cap was removed and/or loosened.

A worker, employed by Satin Finish Hardwood Flooring, Limited (now known as Prodtor Inc.), a manufacturer of pre-finished hardwood flooring of Toronto died after becoming trapped in an operating wood drying kiln.
