On October 28, 2016, a beam being lifted into the air became unbalanced and slid, striking one of the workers and causing critical injuries to the worker. The accident occurred at the Service Mold + Aerospace Inc.’s plant in Windsor, Ontario, where the worker’s employer, CMF Group Inc., was installing new milling machines.

On July 21, 2016, a worker was fatally injured after being crushed by a steel coil that fell off a production line at the Janco Steel Ltd. steel plant in Stoney Creek, Ontario.

A steel truss fell off a forklift onto a worker, causing crushing injuries. An operator should have attended the forklift while its load was raised and the load of steel trusses should have been secured from tipping.

Workers were moving a mobile home at the Golden Pond RV Resort, 4340 Cromarty Drive, RR 2, Mossley, Ontario, a mobile home and recreational vehicle residential park and campground, from one site to an adjacent one when the mobile home shifted, fell off the jacks supporting it, and came down on one of the workers, who died from the injuries.
