A worker, employed by National Steel Car Limited was fatally injured after being struck by a lift-assist device that fell from an overhead crane. National Steel Car Limited failed, as an employer, to ensure that the hoisting controls for the overhead crane were located so they could be operated at a safe distance from a load being lifted. The plant was in Hamilton, Ontario.
