Maple Reinders Construction Ltd. was fined $55,000 on November 29, 2012, for a worker injury that occurred in 2010. Workers were stripping forms off a concrete wall at a waste processing facility under construction in Guelph, Ontario, when the form tipped over, causing the worker to fall from the top of the wall and sustain leg injuries.

A director of ADN Stucco Ltd., a company based in Mississauga, has been fined $3,000 after pleading guilty to failing to ensure that workers using elevating work platforms used safety belts to protect them against falls.

J.R. Contracting Property Services Ltd., a Woodbridge company engaged in garbage removal and hauling, and a supervisor have been found guilty of failing to ensure the safety of a worker who fell off a roof. The company was fined $75,000 and the supervisor has been sentenced to 45 days in jail. A company representative has also been fined $2,000 for obstructing a Ministry of Labour investigation.

Lakehead Roofing & Metal Cladding Ltd., a Thunder Bay roofing company, was fined $50,000 for a violation of the Occupational Health and Safety Act after a worker was injured.
