A worker, employed by Liquor Control Board of Ontario (LCBO), 55 Lakeshore Blvd. East, Toronto, Ontario, sustained a critical injury while operating a lift truck during a training session at an LCBO warehouse on 955 Wilton Grove Road, London, Ontario.

A worker was critically injured while moving a heavy metal beam at the Facility. Misteelco Inc. failed, as an employer, to ensure the beam was handled without endangering the safety of a worker as prescribed by section 45(b) of the Regulation for Industrial Establishments and contrary to section 25(1)(c) of the Occupational Health and Safety Act.

Contrary to safety procedures, 2671475 Ontario Inc. failed, as an employer, to ensure a worker was protected by means of a signaller as outlined in the Occupational Health and Safety Act.
