Regional Sewer and Watermain Ltd. failed to ensure the worker did not use the bucket of the front-end loader as a work platform, which is prohibited by section 107 of Ontario Regulation 213/91 under the Occupational Health and Safety Act.

A worker, employed by the Corporation of the Municipality of Lambton Shores, in Forest, Ontario, was injured while trying to clean a Zamboni ice re-surfacer. The Corporation of the Municipality of Lambton Shores failed to provide information, instruction and supervision to a worker with respect to the safe operation, maintenance and/or cleaning of the ice re-surfacer, contrary to section 25(2)(a) of the Occupational Health and Safety Act.
