A worker was fatally injured during the erection of retaining walls at a construction site for a four-storey apartment building. Contrary to safety procedures, 2671475 Ontario Inc. failed, as an employer, to ensure a worker was protected by means of a signaller as outlined in the Occupational Health and Safety Act.

A worker, employed by Rapid Precision Machining & Gearing Ltd. of Kitchener, Ontario, was injured while operating a metal lathe. Rapid Gear failed as an employer to provide information, instruction and supervision to a worker at a workplace contrary to sections on the Ontario Occupational Health and Safety Act (OHSA).

Contrary to safety procedures, Elmira Pet Products Ltd. failed, as an employer, to ensure a worker was protected by means of a guardrail system or protective coverings as outlined in the Occupational Health and Safety Act.
