Richvale York Block Inc. failed to ensure there was a guardrail at the open side of a raised platform on which a worker stood, as prescribed by section 13(1)(b)(i) of the Regulation for Industrial Establishments. This is contrary to section 25(1)(c) of the Occupational Health and Safety Act, and an offence pursuant to subsection 66(1) of the Act.

A worker was critically injured falling from a height during the maintenance and repair of an oxygen furnace. Contrary to safety measures and procedures prescribed by regulations and the Occupational Health and Safety Act, B. Phillips Company (1987) Limited failed to prevent access to the hazardous area or provide a guardrail and/or fall protection devices for the protection of workers.

Stelumar Advanced Manufacturing (Milton) Inc., a Milton house assembler, was fined $65,000 on July 20, 2009, for violations under the Occupational Health and Safety Act (OHSA) after a worker was seriously injured.
