EllisDon Forming Ltd., out of London, Ontario, and two supervisors were fined after two separate roof collapses occurred during the installation of composite floor systems at two construction projects, causing minor injuries to workers. The company failed, as an employer, to ensure that the measures and procedures prescribed by section 31(1)(b) of Ontario Regulation 213/91 were carried out, contrary to section 25(1)(c) of the Occupational Health and Safety Act. Additionally, Corey Jones and Matthew Thompson as supervisors, contravened section 27(1)(a) of the Act.

A worker, employed by E.H.E. Construction Group Inc. of King, Ontario, was critically injured when a section of a concrete basement wall collapsed at a residential construction project. E.H.E. Construction Group Inc. failed to ensure that the measures and procedures prescribed by section 31(1)(b) of Ontario Regulation 213/91 were carried out at the workplace, contrary to sections 25(1)(c) and 23(1)(c) of the Occupational Health and Safety Act.

The company failed, as an employer, to ensure that the measures and procedures prescribed by section 150(2)(a) of Ontario Regulation 213/91 (Construction Projects) were carried out, contrary to section 25(1)(c) of the Occupational Health and Safety Act.

A worker, employed by Glencore Canada Corporation of Toronto, Ontario, a mining, recycling and critical minerals company involved in mining operations, suffered critical injuries from a fall after Glencore Canada Corporation committed the offence of failing, as an employer, to take every precaution reasonable in the circumstances for the protection of a worker, contrary to section 25(2)(h) of the Occupational Health and Safety Act.
