A workplace fatality, occurred on June 14, 2021, when a large flywheel from a motor suddenly became disconnected from a hub and struck a worker. Western Mechanical Electrical Millwright Services Ltd. (the Company) failed as an employer to comply with section 45(a) of Regulation 851/90, the Regulation for Industrial Establishments.

A driver, employed by Global Metal Sources Co. Ltd., was fatally injured when the box from an adjacent 45-foot dump trailer fell on their trailer cab as the box was being lifted. The company had failed to ensure the dump trailer was maintained in good condition.

A worker, employed by Bernt Gilbertson Enterprise Limited of Richards Landing, Ontario, was critically injured when a motor fell during maintenance work being performed on a portable cone crusher. Gilbertson Enterprises failed as an employer to take every precaution reasonable in the circumstances for the protection of a worker. The accident took place on May 15, 2021.
