An employee of Vinyl Window Designs Ltd., 550 Oakdale Road, North York, Ontario, was killed after entering a barrier enclosure to troubleshoot a machine that had not been properly locked out.

A worker, employed by Norbord Inc., of Toronto, Ontario, an international company that produces wood-based products, including Oriented Strand Board (OSB), a type of engineered wood, was injured when a machine activated unexpectedly. The machine was not equipped with a guard or other device in place to prevent access by a worker to hazardous moving parts.

Give and Go Prepared Foods Corporation, a manufacturer of baked goods, had an employee suffer permanent injury after reaching into a sugar-sifting machine while it was operating. The accident happened at the company’s facility at 725 Intermodal Drive, Brampton, Ontario on June 13, 2017.
