On February 1, 2023, a worker, employed by Forages Technic-Eau Inc. / Technic-Eau Drillings Inc., of Sherbrooke, Quebec, was unloading boxes from an idling crawling carrier that was parked beside a pickup truck. The worker failed to apply the parking brake before exiting the crawling carrier. When exiting the vehicle, the worker then accidentally made contact with its control levers.

A worker employed by Woodstock Ford, a car dealership and service centre, was fatally injured after falling from a stepladder. The company failed to take the reasonable precautions of ensuring that the worker worked from a ladder or platform designed to be used at the required working height, contrary to section 25(2)(h) of the Occupational Health and Safety Act.

Can Art Aluminum Extrusion Canada Inc. failed to ensure the measures and procedures prescribed under section 24 of the Regulation for Industrial Establishments were carried out at the workplace, contrary to section 25(1)(c) of the Occupational Health and Safety Act.
