A worker, employed by AM Wholesale Meats Ltd. of Mississauga, Ontario, a poultry processing and distribution facility, was injured by a falling pallet while operating a pallet truck in the shipping and receiving area. AM Wholesale Meats Ltd. failed to ensure that the measures and procedures prescribed by section 45(b)(ii) of Ontario Regulation 851 were carried out at the workplace, contrary to sections 25(1)(c) and 66(1) of the Occupational Health and Safety Act.

 Gary D. Robinson Contracting Ltd. of St. Thomas, Ontario, failed to ensure that steel trusses were transported, placed or stored in a manner that would not tip, collapse or fall, as required by section 45(b)(i) of Regulation 851/90, a violation of section 25(1)(c) of the Occupational Health and Safety Act.
