A worker was injured while replacing the hydraulic cylinders on a scissor lift when the lift’s platform and lifting mechanism collapsed. Amco Farms Inc., a company that grows, packages and ships produce,  failed to ensure that the platform and lifting mechanism were securely and solidly blocked to prevent the equipment from falling or moving, contrary to section 74 of Ontario Regulation 851 under the Occupational Health and Safety Act.

Bert’s Truck and Trailer Repair Inc. failed, as an employer, to protect a worker by ensuring the safety measures and procedures required for preventing an elevated vehicle from falling on a worker were carried out, as outlined in section 74 of the Regulation for Industrial Establishments.

While replacing the left hydraulic lift cylinder of the arm of a front-end loader, a worker, employed by Finn Way General Contractor Inc., of Thunder Bay, Ontario, was fatally injured when the loader arm dropped after the hydraulic cap placed on the hydraulic lines was removed, releasing the pressurized fluid in the right hydraulic lift cylinder that was holding up the weight of the arm. Finn Way General Contractor Inc., failed as an employer, to ensure measures and procedures prescribed by the Industrial Establishments’, Ontario Regulation 851/90 were carried out in the workplace.
