A worker was injured falling from a platform. Delta Elevator Company Limited failed to provide a guardrail, as required under section 26.1(1) of the Regulation for Construction Projects, contrary to section 25(1)(c) of the Occupational Health and Safety Act.

A worker was injured after falling 15 feet through an HVAC opening in a roof. H&N Roofing & Sheet Metal Limited failed to ensure the worker was protected by appropriate fall protection and wearing protective headwear, as required by sections 26.1(2) and 22 of Ontario Regulation 213. This is contrary to section 25(1)(c) of the Occupational Health and Safety Act.

A worker was injured while setting up a milling machine to manufacture fountain nozzles. Crystal Fountains Inc. failed to ensure that the interlocks on the safety doors of the milling machine were maintained in good condition, contrary to section 25(1)(b) of the Occupational Health and Safety Act.
