On Oct. 15, 2018, a worker employed by Welldone Inc. of Toronto, Ontario, a company that specializes in restoration work, fell from the roof of the building and was killed by the fall. The worker was not protected by a guardrail or travel restraint system as required by law.

A worker was observed by a Ministry of Labour inspector working on a pitched roof without wearing fall protection as required by law. Fall protection contraventions are considered to be one of the ‘killer contraventions’ within the Province of Ontario and are treated extremely seriously by the Ministry of Labour.

The proprietor of a Dorchester-based roofing company was sentenced on November 03, 2016, to serve 3 days in prison and pay $5,000 fine over an incident in which an employee was injured in a fall last year.

Following guilty pleas, Oskam Welding (2014) Ltd. and one of its supervisors, Chad Wheeler, have been fined a combined total of $130,000 after a worker was killed in a fall from the structure the worker was assembling.
