A worker, employed by Regional Sewer and Watermain Ltd., of Cambridge, Ontario was injured after falling from the bucket of a front-end loader. Regional Sewer and Watermain Ltd. failed to ensure the worker did not use the bucket of the front-end loader as a work platform, which is prohibited by section 107 of Ontario Regulation 213/91 under the Occupational Health and Safety Act.

A worker, employed by the Corporation of the Municipality of Lambton Shores, in Forest, Ontario, was injured while trying to clean a Zamboni ice re-surfacer. The Corporation of the Municipality of Lambton Shores failed to provide information, instruction and supervision to a worker with respect to the safe operation, maintenance and/or cleaning of the ice re-surfacer, contrary to section 25(2)(a) of the Occupational Health and Safety Act.

A worker, employed by Sin Structures Inc. of Brampton, Ontario, was critically injured during the construction of a bridge. Contrary to safety procedures, Sin Structures Inc. failed, as an employer, to ensure equipment was moved without endangering the safety of a worker as outlined in the Occupational Health and Safety Act.
