A worker, employed by Bellai Brothers Construction Ltd. of Ottawa, Ontario, a concrete contractor, was injured when moving a fly form, a system used for supporting poured concrete slabs on buildings during construction. The company failed to ensure the fly form was stored and moved in a manner that did not endanger the worker, as required by section 37(1) of Ontario Regulation 213/91, and contrary to section 25(1)(c) of the Occupational Health and Safety Act.

A worker, employed by Valley Blades Ltd., Waterloo, Ontario, a company that manufactures blades, cutting edges and other accessories for construction, mining and snow removal equipment, was injured after their clothing became entangled with operating machinery. The company failed, as an employer, to prohibit the worker from wearing loose clothing around a source of entanglement, as required by section 83(2) of Ontario Regulation 851/90, contrary to section 25(1)(c) of the Occupational Health and Safety Act.

A worker, employed by AMI Attachments Inc. of Hawksville, Ontario, a heavy construction equipment manufacturer, was seriously injured while operating a magnetic lifting device. AMI Attachments Inc. failed to assign a competent person to operate the lifting device, or to ensure the operator was instructed and accompanied by a competent person, as prescribed by section 51(2) of Ontario Regulation 851/90, contrary to sections 25(1)(c) and 66(1) of the Occupational Health and Safety Act.

A worker, employed by Compass Minerals Canada Corp., of Mississauga, Ontario, a salt mining company. was critically injured while assisting with repairs on a flexible conveyor train. By failing to close off the area and position warning signs to protect workers, Compass Minerals Canada Corp. failed to ensure that the measures and procedures as prescribed by section 68 of Ontario Regulation 854/90 were carried out at the workplace, contrary to section 25(1)(c) of the Occupational Health and Safety Act.
