A worker, employed by John Harris Concrete Ltd. of Chatham, Ontario, a very small construction company specializing in concrete foundations, was fatally injured while cutting down a tree. The company failed, as an employer, to provide training to the worker as required by section 25(2)(a) of the Occupational Health and Safety Act.

Siltech Corporation, a chemical manufacturing company, failed, as an employer, to ensure containers were bonded and grounded as required by section 22(4)(b) of Ontario regulation 851/90, contrary to section 25(1)(c) of the Occupational Health and Safety Act.

A worker, employed by Canadian General, of Cambridge, Ontario, a company that produces polymeric coated textile fabrics and vinyl films for automotive and industrial applications, was injured after being caught in the pinch point of a machine. The company failed to ensure the worker was protected from the pinch point, as required by section 25 of Ontario Regulation 851/90, contrary to section 25(1)(c) of the Occupational Health and Safety Act.

Ryan Wilkes is a project supervisor employed by The Lake Partnership Inc. A worker fell through an opening in the floor of a home under construction. Mr. Wilkes failed, as a supervisor, to ensure that the worker was protected by a guardrail or protective covering as required by section 26.3(2) of Ontario Regulation 213/91, contrary to section 27(1)(a) of the Occupational Health and Safety Act.
