A coroner’s jury probing the death of an Ontario construction worker in 2006 has made a series of recommendations meant to enhance fall protection.

BFI Canada Inc., a waste management company operating in Canada and the United States, was fined $150,000 for a violation of the Occupational Health and Safety Act after a worker was injured.
On May 28, 2009, a worker from a temporary help agency was assigned to a waste collection route for BFI Canada Inc. in Ottawa. The worker’s job was to take recyclable material from the curb and put it into a waste collection truck. While performing these duties, the worker got out of the truck while it was still moving and the truck ran over the worker’s foot.

Cimco Refrigeration, a division of Toromont Industries Ltd. that manufactures and maintains refrigeration systems, was fined $95,000 for a violation of the Occupational Health and Safety Act after a worker was injured. Sobeys Capital Inc., carrying on business as Sobey’s Whitby Retail Support Centre, a grocery distribution centre, was fined $30,000 in relation to the same incident.

Kirkland Lake Gold Inc., a company that operates an underground gold mine in Kirkland Lake, was fined $100,000 on September 6, 2011, for violating the Occupational Health and Safety Act after two workers were injured.
