Roofing workers removed a skylight but did not install a guardrail system around the opening or place a protective covering over the opening as required by safety regulations. A worker fell through the skylight opening.

Pro Renovations Solutions Inc. of Mississauga, was subcontracted to make repairs to an apartment building in Peterborough, Ontario. On November 01, 2016, a worker on a 10-storey building lost balance at the roof edge and fell to the ground, sustaining critical injuries as a result. The worker was not protected by any form of fall protection while on the roof.

During the work day of September 25, 2016, workers on a job site were observed not wearing fall protection (such as harnesses or hard hats) as required by Ontario Regulation 213/91, the Construction Projects Regulation.

On December 11, 2015, a young worker fell from the roof of a house that was being re-shingled. A lanyard was apparently detached from a safety line at the time. The young worker was hospitalized for over six months but did not regain consciousness and passed away.
