Hypoxia, or in-flight oxygen deprivation, was a likely factor in a plane crash that killed a pilot and survey technician in Alberta last year, according to a recent investigation report from the Transportation Safety Board of Canada (TSB).

At the Detour Lake Gold mine, about 185 km northeast of Cochrane, Ontario, on August 24, 2014, A worker hauling rocks in a dump truck being operated in reverse was critically injured when the truck fell and flipped over on its roof. There was no berm (protective barrier) to support the truck and the road gave way beneath the truck.

At Consolidated Fastfrate Inc., a national provider of logistic services, including shipping and warehousing, on February 12, 2016, A worker who was operating a forklift struck another worker who could not be seen because the forklift operator had an obstructed view of the forklift’s path of travel. The injured worker suffered a permanent injury.
